Southwestern Vermont Medical Center's (SVMC's) interactive web portals give patients and their families easy, secure access to their health information. The portals also provide quick, online communication with staff. Because the information comes directly from your medical record, you can be sure it’s up-to-date.
SVMC uses two separate portals:
- Hospital Patient Portal: Use this portal to access information related to hospital services you received
- SVMC Practices and Affiliates Athena Patient Portal: Use this portal to access information related services you received at SVMC medical practices and offices
Hospital Patient Portal
The Hospital Patient Portal has information in your hospital medical record, including information from:
- Your inpatient stay
- Your emergency department visit
- Laboratory tests
- X-ray or other imaging tests
- Sleep testing
- Other tests conducted at SVMC
Use the Hospital Patient portal to:
- View lab results
- Find a list of current medications and allergies

Enrolling in the Hospital Patient portal
There are two ways to enroll in the Hospital Patient Portal:
- When you are registering for an appointment, add your email address to your medical record. Ask for a portal instruction pamphlet with your medical record number inside.
- Call 802-447-5687 to add your email address to your medical record and be provided your medical record number.
Once your email address is filed, you will be auto-enrolled and will receive an email from the Patient Portal with further instructions for setting up your account.

The SVMC Practices and Affiliates Athena Patient Portal has information from SVMC's medical practices, including:

- Dentistry
- Dermatology
- ExpressCare
- General Surgery
- Infectious Disease
- Internal Medicine
- Medical Oncology
- Multispecialty
- Neurology
- Northshire Campus
- Obstetrics and Gynecology
- Orthopedics (including Northern Berkshire Orthopedics)
- Pediatrics
- Pownal Campus
- Psychiatry
- Pulmonology
- Radiation Oncology
- Rheumatology
- Twin Rivers
Use the SVMC Practices and Affiliates Athena Patient Portal to:
- View lab results
- Find a list of current medications and allergies
- Send a secure message to your provider's office

Enrolling in the SVMC Practices and Affiliates Athena Patient Portal
To enroll in the SVMC Practices and Affiliates Athena Patient Portal, you will need to go to your physician's office and obtain a letter with PIN number. For more information, please call 802-440-6000, option 2.
Accessing a relative's information
In addition to handling your own care, SVMC allows parents and healthcare proxies with authorized consent to access the records of those individuals for whom they manage care.
You can request proxy access for the portal by completing the appropriate form below and returning to the Health Information Services department. For questions or additional information, please call 802-447-5687.
