Treating patients 18 years and older, our physicians and advanced practice providers integrate a patient-centered approach to primary care with advanced technology and exceptionally high standards. We’ll get to the root of your symptoms and provide the testing, treatment and care you need, all under one roof.

A patient-centered medical home
Recognized by the National Committee for Quality Assurance (NCQA) as a patient-centered medical home, we provide a new model of personal, efficient, and effective primary care that puts patients at the forefront of care. Our community health team includes a diabetic educator, registered dietician, and community health programs.

A provider is on call for urgent (non-emergent) issues 24 hours a day. When the office is closed, an answering service will quickly relay your message to the provider on call. After you have made contact with the answering service, leave your phone line open so the on-call provider may return your call without delay. In the event of an emergency, please call 911.
