A patient-centered medical home
Recognized by the National Committee for Quality Assurance (NCQA) as a patient-centered medical home, we provide a new model of personal, efficient, and effective primary care that puts patients at the forefront of care.
Our community health team includes a diabetic educator, mental health and substance abuse counselor, nurse case manager, and registered dietician.

A provider is on call for urgent (non-emergent) issues 24 hours a day. When the office is closed, an answering service will quickly relay your message to the provider on call. After you have made contact with the answering service, leave your phone line open so the on-call provider may return your call without delay.
